| Step | Hazards | Controls |
|---|---|---|
| Assess the task against the height hierarchy | Inappropriate ladder use for sustained or higher-risk work, falls from height | Confirm at the prestart that the task is low risk and short duration; where it is not, require a platform or Main Contractor provided scaffold |
| Select and inspect the ladder | Defective, domestic-rated or wrong-length ladder | Industrial rated only, pre-use inspection, tag out and remove defective ladders immediately, correct length for a 1m hand-hold above the work |
| Set up and secure | Slip-out, soft or sloping ground, doors opening into the ladder, vehicles or people | 4:1 pitch, level and firm footing with levellers, tie off or foot the ladder, control doors and traffic, barrier and sign the base |
| Work from the ladder | Falls, dropped tools, over-reaching, electrical contact | Three points of contact, belt or hoist tools, keep the body within the stiles, non-conductive ladders near electrical hazards, descend and reposition rather than reach |
